Risk of dying of prostate cancer really has been decreasing ? THE ...

According to a study published on line last week in the Journal of the National Cancer Institute, not only are men with prostate cancer more likely to die from other conditions, the risk that they will die of prostate cancer (as opposed to other conditions) has been decreasing significantly over time.

The study by Epstein et al. is based on data from > 700,000 men identified in the U.S. Surveillance, Epidemiology, and End Results (SEER) and in the Swedish Cancer and Cause of Death databases. According to the lead researcher (as quoted in an article on the HealthDay web site): ?Our study is the first to analyze specific causes of death among men with prostate cancer.?

The databases allowed the research team to look at causes of deaths from 1973 through 2008 for 490,341 men in the USA and from 1961 through 2008 for 210,212 men in Sweden.

The authors were then able to estimate the cumulative incidence of death from seven selected causes accounting for > 80 percent of the reported deaths and also to analyze mortality trends by calendar year and age at diagnosis and length of follow-up.

There primary results are reported as follows:

  • Among all the men in the two databases
    • 52 percent of the men in Sweden died during the 47-year study period.
    • 30 percent of men in the USA died during the 35-year study period.
  • Among men diagnosed with prostate cancer in the two databases
    • 35 percent of Swedish men died from this disease during the study period.
    • 16 percent of American men died from this disease during the study period.
  • The cumulative incidence of prostate cancer-specific death declined during follow-up in both populations.
  • The cumulative incidences of death from ischemic heart disease and from cancers other than prostate cancer remained constant.
  • During the last 5 years of the study period (i.e., from 2003 to 2008),
    • The 5-year cumulative incidence of death from prostate cancer was 29 percent in Sweden.
    • The 5-year cumulative incidence of death from prostate cancer was 11 percent in the USA.

It is also noted that most of the men who died from prostate cancer during the two study periods were men diagnosed when they were older or were diagnosed before the widespread use of the PSA test.

Dr. Epstein is further quoted as stating:

We hope the study will have an impact on the clinical management of men who receive a diagnosis of prostate cancer.

and

We hope it will encourage physicians to use the diagnosis as a teachable moment to encourage men to modify lifestyle factors, like losing weight, increasing physical activity and stopping smoking. We believe that adopting a healthier lifestyle may reduce a man?s risk of other chronic medical conditions that ultimately account for more deaths among men with prostate cancer than the disease itself.

The report on the HealthDay web site also offers statements from Dr. Durado Brooks of the American Cancer Society and from Dr. Louis Kavoussi, chairman of urology at North Shore-Long Island Jewish Health System in New York, with regard to the implications of these data on the need for appropriate use of the PSA test (as opposed to mass, population-based screening). According to Kavoussi:

Prostate cancer does kill and if you pick it up early, it can be treated. The problem with PSA screening is that too many men are treated when they don?t have to be. It?s not bad to detect prostate cancer. The bad thing is that you treat everybody.

He added that:

Many men don?t need to be screened. Those who should be screened are men with a family history of prostate cancer, blacks and anyone else at high risk.

Epstein and her colleagues conclude that, in both the USA and Sweden:

  • Men diagnosed with prostate cancer are less likely to die from prostate cancer than from another cause, and that
  • Because many of these other causes of death are preventable through changes in lifestyle, interventions that target lifestyle factors should be integrated into prostate cancer management.

Are the World's Reptile Species in Trouble?

Dear EarthTalk: How are the world's reptile species faring in terms of population numbers and endangered status? What's being done, if anything, to help them?? Vicky Desmond, Troy, N.Y.

The world?s reptiles?turtles, snakes, lizards, alligators and crocodiles?are indeed in trouble. The International Union for Conservation of Nature, which publishes an annual global roster of threatened and endangered species called the Red List, considers some 664 species of reptiles?representing more than 20 percent of known reptile species worldwide?as endangered or facing extinction. Meanwhile, the U.S. Fish & Wildlife Service considers about 10 percent of American reptiles threatened or endangered.

Why care? The non-profit Center for Biological Diversity (CBD) considers reptiles ?amazing creatures? with clever adaptations that have helped them survive for millions of years. CBD also points out that reptiles are valuable indicators of wider ecological health. ?Because many reptile species are long-lived and relatively slow-moving, they suffer from disturbances like habitat loss or pollution for extended periods,? the group reports, adding that a diverse community of reptiles living in a given area is evidence of a healthy ecosystem that can support the plant and animal life they and other species need for food and cover.

So what?s causing the reptiles? decline? ?While habitat loss is the most obvious cause of endangerment, declines are even even occurring in pristine areas from threats such as disease, UV radiation and climate change,? reports CBD. Overcollecting and unregulated hunting also are taking a toll on reptile populations.

In order to help stem the tide of reptile loss, CBD leverages the court system to pressure the federal government to protect at-risk species. For instance, back in 2004 the group worked with the Coalition for Sonoran Desert Protection in filing a petition to add the Tucson shovel-nosed snake, which dwells in the quickly disappearing wild desert around fast-growing cities like Tucson and Phoenix, to the federal list of endangered species. Finally in 2011 the federal government agreed that it would add the snake to its list of endangered species which will help it get the habitat protection needed to ensure long term survival.

CBD also works on other fronts for reptiles. The group?s campaign to outlaw ?rattlesnake round-ups??contests whereby hunters collect and kill as many snakes as they can in a year?has helped stem population declines of eastern diamondback rattlesnakes. And CBD?s efforts to educate the public about the plight of freshwater turtles, which are ?overcollected? for food and the pet trade in the southern and midwestern parts of the U.S., helped convince several states for the first time to regulate turtle harvests.

One way everyone can help reptile species in decline is to make our backyards friendly to them. The U.S. Geological Survey?s Patuxent Wildlife Research Center offers tips on what to plant and how to arrange a landscape to encourage reptiles and other wildlife. Landowners that take these steps may be rewarded with fewer pests, given reptiles taste for large numbers of mosquitoes and other insects as well as small rodents. Other pro-reptile tips include driving carefully (road mortality is a big issue for snakes, turtles and other species) and keeping outside areas around your property free of garbage that might attract raccoons, crows and other pests that also prey on reptiles.

Rare Diseases: 5 Recent Reasons to Cheer

On Sunday morning, July 21, I faced a room of people from families with Leber congenital amaurosis (LCA), an inherited blindness caused by mutations in any of at least 18 genes. It was the final session of the Foundation for Retinal Research?s bi-annual LCA family conference, and I was there to discuss the history of gene therapy. But I zapped through that quickly, because the future is much more intriguing.

The excitement pervading the room that day was palpable, following a day of scientific updates, and not only because those with young children were soon to visit Sesame World and the sights of Philadelphia.

Jennifer and Troy Stevens exemplified that hope. Two years earlier, at this conference, they?d learned that researchers had been unable to identify a mutation behind their toddler Gavin?s blindness. Now they know the name of their gene: NMNAT1. I?ll return to their story.

The star of the 2010 conference had been 10-year-old Corey Haas and an energetic young sheepdog, both cured of LCA with gene therapy. This weekend, the stars were the new programs and technologies that would allow other families to join Corey?s ? and not just those with blindness.

The rare disease community in the US collectively belies its name: at least 30 million people suffer from 7,000+ diseases, many so rare that they hover beneath the radar of big pharma. But maybe not for long, thanks to the following recent reasons to cheer:

#1: GENE THERAPY PENDING APPROVAL

On July 20, the European Medicines Agency (EMA) announced impending first approval of a gene therapy in the western world.

It?s for lipoprotein lipase deficiency (LPLD). The enzyme normally breaks down tiny triglyceride-packed globules called chylomicrons, and its absence causes episodes of very painful pancreatitis that can be fatal. LPLD is an ultra-rare disease, striking 1-2 people per million. And the only treatment is a diet so low in fat that most patients can?t stick to it.

The gene therapy, Glybera, consists of adeno-associated virus type 1 delivering an overactive variant of the LPL gene, injected into a leg muscle during a single day. But not many people have had it.

The research team, led by Daniel Gaudet, MD, PhD, a professor of medicine at the University of Montreal, with colleagues from Amsterdam Molecular Therapeutics (recently replaced by privately-held UniQure), reported a two-year follow-up of 14 adult patients receiving 100 billion to 1 trillion viruses. And it seems to have worked, depending upon how one assesses success.

?The triglycerides dropped, but after 60 days they trended back up. The primary endpoint had failed, but the secondary endpoint was recurring episodes of pancreatitis ? and they found a statistically significant, or close to it, decrease,? explained James Wilson, MD, PhD, editor-in-chief of Human Gene Therapy and professor of pathology and laboratory medicine at the University of Pennsylvania, who developed the vector. Tracking a few more patients, work not yet published, may have led the EMA?s Committee for Medicinal Products for Human Use to finally recommend approval, after three rejections.

Tomas Salmonson, MD, acting chair of the committee, points to the new data as well as restricting use to the sickest patients in pushing the gene therapy forward. ?Our established ways of assessing the benefits and risks of Glybera were challenged by the extreme rarity of the condition and also by uncertainties associated with data provided.?

For the additional study, the researchers looked at what was happening in the chylomicrons in the blood, and found that triglyceride level can fluctuate, contrary to assumptions of steady change. And that means something is happening that might explain the decrease in the painful episodes ? a very real measurement. Summed up Jean Bennett, MD, PhD, leader of one of the LCA2 clinical trials at Penn, ?It?s a huge vote of confidence for the entire field of gene transfer.?

Dr. Wilson agrees. The repercussions won?t be at the FDA, where scientists make decisions based on data, he said, but on the willingness of big pharma to invest in gene therapy. Despite recent successes ? LCA2, hemophilia, adrenoleukodystrophy ? the pharmaceutical industry has been hesitant to fund gene therapy because it has lacked an approval. ?So-called regulatory uncertainty has been the biggest problem, and if there?s no precedent, they can continue to say no. Biopharma is not interested in the ultra orphans. But I have a feeling we?ll be seeing some activity,? he added.

#2: FINDING HOMES FOR STALLED DRUGS

By August 14, researchers can submit pre-applications to the National Center for Advancing Translational Sciences (NCATS) Discovering New Therapeutic Uses for Existing Molecules program. The idea is simple yet brilliant: match compounds that are languishing on company shelves to diseases with newly-discovered mechanisms. Such candidate drugs have passed initial safety tests but were dropped for business reasons, such as a tiny market, or because they didn?t treat what they were intended to.

Since the announcement in June, eight industry leaders have signed on, offering an initial 58 compounds to find new therapeutic homes. And the need is compelling: of the 4,500+ diseases with recently-revealed mechanisms, only about 250 have treatments. ?If researchers funded through this effort can demonstrate new uses for the compounds, they could significantly reduce the amount of time it takes to get a treatment to patients in need,? said Kathy L. Hudson, PhD, NCATS acting deputy director.

Everyone wins.

#3: SPEEDING FDA APPROVAL

On July 9, President Obama signed into law the FDA Safety and Innovation Act, which updates the 1983 Orphan Drug Act. The new law provides $6 billion over the next 5 years to assist the agency in evaluating new drugs and medical devices. The Act will speed access to new treatments and development of especially promising ones, and the Humanitarian Use Devices program will target those that treat rare diseases, giving priority to diseases of children. ?Treatments are desperately needed because most are serious, many are life-threatening, and about two-thirds of the patients are children,? said Peter L. Saltonstall, president and CEO of the National Organization for Rare Disorders (NORD), which was critical in developing both acts.

The Act may be a lifesaver for people such as 8-year-old Hannah Sames, one of 54 people in the world known to have giant axonal neuropathy. The gene therapy trial that she will take part in is nearing phase 1, but the sponsoring not-for-profit, Hannah?s Hope Fund, is about to run out of money.

#4: EASING INSURANCE ACCESS

When the Supreme Court upheld the Affordable Care Act on June 28, I scrolled through the relieved statements from various rare disease organizations. Thanks to the ACA, children like Hannah Sames and Gavin Stevens will not be penalized for their pre-existing conditions, nor face annual or lifetime insurance caps.

#5: IDENTIFYING DISEASE GENES

Exome sequencing can identify mutations when single-gene tests don?t. The strategy sequences the protein-encoding part of the human genome in individuals, usually young children, whose syndrome has evaded recognition, searching for mutations passed silently from parents, with functions that could explain the symptoms. Once that?s known, researchers can develop new treatments, or repurpose existing ones.

New exome-derived discoveries are being reported nearly weekly, some appearing in the media before the technical papers are published. A recent news release about a 4-year-old named Maya with a neurological disease, for example, made its way into many news reports and blogs, with a touching story and accolades. Yet none named the gene or its precise function ? the part I?m most interested in.

In contrast to the incomplete Maya story, when John Chiang, PhD, director of the Molecular Diagnostics Laboratory at the Casey Eye Institute in Portland, Oregon told me he?d discovered Gavin Stevens? mutation among nearly 2,500 gene variants in the blind boy?s exome, he asked that I not report it. That was 8 months ago ? the mutation is unveiled in a quartet of papers in the current Nature Genetics, after something of a turf war among four research groups.

Gavin?s parents had heard about Dr. Chiang at the Foundation for Retinal Research meeting two years ago, where Jennifer had called him, distraught, after learning that single-gene tests couldn?t explain their son?s blindness. Dr. Chiang, who described his skill as ?I do the dirty work, I find the mutations,? had helped several families after existing tests had fruitlessly, but expensively, probed the most common parts of only the most common genes. Dr. Chiang had first developed larger gene testing panels, and when those still didn?t identify some families? mutations, quietly sent their DNA off to the Beijing Genome Institute for whole exome sequencing.

Now that exome sequencing is commercially available in the U.S., Dr. Chiang cautions that it still doesn?t help all families, and that costs can greatly exceed the oft-mentioned $1,000 pricetag when considering analysis. ?I would only recommend it as the last resort when all known genes are ruled out,? he advised.

On Saturday at the retinal research conference last weekend, I watched Jennifer and Troy beam as Eric Pierce, MD, PhD, director of the Ocular Genomics Institute in Boston and co-author of one of the Nature Genetics papers, talked about their mutation. Discovery of the gene, which affects cellular energy (NAD synthesis), is a starting point for gene therapy, and this particular candidate is a great target. ?The gene is small, and encodes an enzyme,? said Dr. Pierce.

The next day, as my talk about the history of gene therapy wound down, I took stock of my audience. Two young women with canes sat in the front row. A few rows back sat Karen Poulakos, also with a cane, whom I?d chatted with earlier.

Karen has Corey?s disease, LCA2, but, at age 63, had been deemed too old for the gene therapy clinical trial two years ago. But things had changed, she?d learned at the meeting, and she just might be eligible for the phase 3 trial coming up. Karen has lived a full life in her world of shadows, barely remembering when she could see better, and she?s now contemplating what it might be like to see again.

As I collected my things, I marveled at the hope radiating from the faces in the room, sighted as well as not. And I thought that this is science at its very best. This is what it is all about, the molecules, the mice, the deciphering of nature?s mechanisms: helping people.

Troops, students, teachers to fill Olympic seats

LONDON (AP) -- Troops, teachers and students are getting free tickets to fill prime seats that were empty at some Olympic venues on the first full day of competition.

Organizing chief Sebastian Coe answered widespread criticism Sunday by predicting that seats left unused, largely by Olympic and sports officials, will not be an issue as the games proceed.

"It is obvious, some of those seats are not being used in the early rounds," he said at a briefing.

He declined to blame Olympic sponsors, whom he had earlier promised to "name and shame" if they did not use their allocations. Sponsors, including Coca-Cola and Visa, defended their use of allotted tickets - 8 percent of the 8.8 million available tickets.

The issue is sensitive for Olympic organizers and British sports fans after hundreds of thousands of people failed to get tickets in an initial public ballot.

"There is not a single person who thinks it is shambolic," Coe insisted, adding no one would object to free tickets for military personnel who "stepped up to the mark" this month to help solve a security staffing crisis at venues.

Coe's organizing team has long promised to fill venues and avoid a similar problem from the Beijing Olympics.

He said he was "jammed in shoulder to shoulder" with Olympic officials to see swimming finals - including the first Ryan Lochte-Michael Phelps duel - on Saturday evening. He also pointed to record crowds lining the men's road race route, and for rowing events at Eton Dorney.

"Those venues are stuffed to the gunnels. The public are in there," Coe said.

Yet broadcast images of signature Olympic events, such as gymnastics and swimming, revealed rows of empty seats for qualifying rounds Saturday. Tennis at Wimbledon's Centre Court was sparsely attended just weeks after the Grand Slam event sold out.

"There are a whole bunch of the military actually sitting in those seats at the moment. We can and we have moved them in there," Coe said.

Students and teachers from east London neighborhoods also would get late calls for free tickets, having already been accredited in a planned reallocation program. Some ticket holders will get upgrades inside venues, Coe said.

Some blame for the opening day embarrassment was falling on "accredited people," including the Olympic family, athletes, and some sponsors and media, organizers said.

Coe said it was typical at Olympics for sports and national team officials to be "dragged to any number of venues," and be too busy to attend events in the opening days. "I don't think you will be seeing this as an issue long term during the games."

He went on to defend sponsors, whose legal rights to protect their brand at the Olympics often fuel criticism.

"I am not sure naming and shaming is what we are into at the moment. Sponsors are turning up," Coe said.

Still, British Culture Secretary Jeremy Hunt had said Saturday that no-shows were suspected to be corporate guests.

"We think it was accredited seats that belonged to sponsors," Hunt said. "But if they're not going to turn up, we want those tickets to be available for members of the public, because that creates the best atmosphere."

Coca-Cola and Visa said they gave most of their allocated seats to prize winners in promotional offers. Coca-Cola said its competitions allowed people "to choose the event they really wanted to attend."

"We have also invited some long-standing partners, employees, and customers to attend the games. We believe that usage levels of our tickets have been extremely high so far," the company said.

Visa said it made "great efforts to ensure that our ticket allocations are fully used."

Organizers will use further tactics to boost attendance and offer more opportunities to see Olympic events.

Releasing tickets for walk-up sales allowed for about 1,000 seats to be sold for gymnastics Saturday, and the Wimbledon policy of "recycling" tickets has been adopted.

Coe said almost 300 people saw handball matches Saturday by buying cheap tickets - 5 British pounds ($7.87) for adults, 1 pound ($1.57) for children - which were handed in for resale by the original ticket holders on leaving the venue.

As I implied earlier, in the early 70s I would've eaten this stuff up like bon-bons because the band's overall musicianship and tightness is nothing short of phenomenal. The royal intro they attach to "Phantom Lord" doesn't last long as they soon revert to their bread-and-butter, unrestricted maniacal rock. Again they surprise me by digressing into a semi-prog motif briefly before returning to their blazing sprint. I found myself amused during "No Remorse" by how they consistently stick Kirk's guitar leads way up front in the mix as if to say "we don't care about being slick, Dick, just listen to THIS!" It's another road-grader of a tune with no hint of a let up in their aural assault to be found. "Seek and Destroy" exemplifies everything ruthless about metal music due to its unapologetically pulsating, head-banging mindset. This cut, maybe more than any other on the disc, forces their regal roots to the surface and shows Metallica to be an ensemble steeped in the grand tradition of the rock titans that were roaring like lions when they were still pooping their Pampers. "Metal Militia" is next and there's only so much you can assimilate when they're flying by at the speed of light. I just tried to hang on. The much slower, menacing approach they adopt for the start of "Am I Evil" brings to mind the dark aura that Black Sabbath once exuded. But by now the unrelenting sledgehammer effect becomes headache-inducing inside this graying head of mine although Hammett does a fantastic job of imitating Ritchie Blackmore on this song. They close with "Blitzkrieg" and they go out the way you'd expect, riding atop another growling riff like demonic cowboys and relentlessly chewing up the landscape without mercy.

When Metallica was added to this site I, not being all that familiar with their body of work, had reservations about their qualifications. However, now that I've investigated their debut, I clearly see the progressive spirit they possess because, while they may be many things, they're never boring, unimaginative or cowardly and those are essential ingredients to being labeled as prog-related. I remember the excitement I experienced every time bands like Trapeze dug in their claws and produced gut-grabbing hard rock in the 60s and 70s that acted as a necessary pressure-release valve for my pent-up emotions. Now I'm starting to understand why Metallica's music meant so much to so many, especially during the empty 80s. They met the same primal need that must be met in each generation, else the Earth will explode. 3.4 stars.

Russia says "not thinking about" asylum for Assad

MOSCOW (Reuters) - Russia has made no agreement to grant Syrian President Bashar al-Assad asylum and is "not even thinking about" doing so, Foreign Minister Sergei Lavrov said on Saturday.

The remarks fell short of a statement that Russia would not consider taking in Assad but were among Moscow's strongest indications yet that it is not planning to do so.

"We have said more than once publicly that we are not even thinking about this," Lavrov said when asked about media reports Russia was ready to offer Assad asylum.

"There is no agreement, no thought about this issue," Lavrov told reporters on a flight to Moscow from the Black Sea resort of Sochi, where he and President Vladimir Putin met Japan's foreign minister.

"This is all a provocation by those who want to place all the blame for what is happening in Syria on us and on China."

Lavrov reiterated statements by Putin and other Russian officials that a Moscow has no special relationship with the Syrian government, suggesting it might make more sense for a Western nation to take him in.

"We are not and have not been the closest friends of the Syrian regime. Its best friends are in Europe, and if somebody wants to resolve this issue in such a way, let them think about their capabilities."

Russia has said it is not propping up Assad and would accept his exit from power in a political transition decided by the Syrian people, but that his exit must be a precondition and he must not be pushed out by external forces.

Romney seeks lift in Israel after London miscues

LONDON -- Mitt Romney will try to rebuild momentum for his closely watched foreign trip Saturday when he arrives in Israel, where the presumptive GOP nominee will look to escape two days' worth of negative headlines plaguing his campaign in London.

Romney will arrive in Jerusalem with hopes of finding surer political footing than in London. He spent most of his two days in the United Kingdom's capital trying to clean up a controversy spurred by his comments to NBC doubting London's preparation for the Summer Olympic Games. He and his wife Ann attended the opening ceremonies last night.


This will be Romney's fourth visit to Israel, and it's expected to be another whirlwind day of meetings with top officials, including Prime Minister Benjamin Netanyahu (a friend of Romney's from their days at Boston Consulting Group decades ago), and Palestinian leader Salam Fayyad, whom Romney also knows from previous visits.

Romney will also give a public speech while in Jerusalem ??his first such engagement thus far during his foreign trip ??that is expected to be heavy on praise for America's most important ally in the Middle East, with whom Romney has repeatedly called for the U.S. to "lock arms" on the international stage.

The presumptive GOP nominee has pledged to keep with diplomatic tradition by not criticizing President Obama or his foreign policy while on foreign soil, but in an interview with the Israeli newspaper Hayom (owned by conservative mega-donor Sheldon Adelson), Romney ripped the president for various elements of his policy toward Israel, including the president's call for the Israel to consider returning to its pre-1967 borders, including land swaps with the Palestinians to compensate for disputed territory.

"The president has also spoken of returning to 1967 borders ? they are indefensible. And acting as a negotiator and usurping the primary role played by Israel in negotiating for its own future is not the right course for America to take," Romney told the paper.

Romney also indicated in his interview with Hayom that his primary audience on this trip to Israel was, after all, American voters. In 2008, President Obama carried the Jewish vote with 78 percent of the vote, a lead into which Romney's campaign would love to make inroads.

Former U.S. Ambassador to Israel Martin Indyk explains:

"The context is a political campaign in which there is a sense that because of tensions between Obama and Benjamin Netanyahu the American Jewish vote is up for grabs," said Indyk, who served under Bill Clinton. "In that context its natural that the Republican campaign would seek to emphasize the friendships Romney has in Israel and the problems Obama has had with Netanyahu."

Indyk, who has authored a book on Obama's foreign policy, described Obama's security record on Israel - enhanced Friday by the signing of a new agreement - "impeccable," but said Romney had room to appeal to Israelis and Americans by showing more warmth to Israel than the sometimes cold Obama.

Sure to be on the agenda in Romney's various meetings: the specter of a nuclear Iran, the most pressing security issue to Israel.

"Again, he's got to walk between the raindrops, because while he'll try to draw distinction and claim Obama is weak and he is strong on this issue, he's got to be careful not to give the American voting public the impression if he's president he'll go to war with Iran," Indyk said. ?

Administration projects $1.2T 2012 budget deficit

WASHINGTON (AP) ? The White House predicts this year's federal budget deficit will end up at $1.2 trillion, marking the fourth consecutive year of trillion dollar-plus deficits during President Barack Obama's administration.

The bleak figures, while expected, are sure to add fuel to the already heated presidential campaign, in which Obama's handling of the economy and the budget is a main topic. Friday's release came as the government announced that U.S. economic growth slowed to an annual rate of just 1.5 percent in the second quarter of this year, as consumers cut back sharply on spending.

The White House budget office also predicts for this year that the economy will grow at a modest 2.6 percent annual rate and that the jobless rate will average 8 percent. It forecasts modest growth of 2.6 percent next year ? down from the 3.0 percent it predicted in February ? before rising to 4.0 percent in 2014. Unemployment would remain above 7 percent through the end of 2014, registering at 7.3 percent, the report predicts.

"The economic recovery that began in 2009 will continue at a moderate rate and unemployment will gradually decline," Jeffrey Zients, the acting White House budget director said in a blog post. "The economy still faces significant headwinds," he added.

The 2012 budget year ends on Sept. 30. The White House also predicted that next year's deficit will fall just short of $1 trillion, higher than it predicted in its February budget release. The predicted deficit for 2012 actually improved by $116 billion, but some of that was because Congress didn't enact much of Obama's jobs plan.

But the White House promises deficits will drop to about 3 percent of the size of the economy by 2017, in part through $1.5 trillion in tax increases over the coming decade.

The White House report ? released Friday afternoon with the Olympics poised to distract voters for two weeks ? again trumpets Obama's longstanding approach to tackling the deficit. It includes tax increases on families earning above $250,000, already enacted "caps" on agency operating budgets and modest savings from federal benefit programs like Medicare and Medicaid.

"Since taking office, the president has worked to restore fiscal responsibility," says the OMB report.

Republicans scoffed, noting that Obama has violated his promise to wrestle the deficit in half by the end of his term.

"Much like his record on jobs, the president's pledge to halve the deficit is a glaring broken promise," said Brendan Buck, spokesman for House Speaker John Boehner, R-Ohio. "This level of debt serves as a major roadblock to economic recovery and threatens our ability to meet the promises we've made to our seniors and most needy."

Under Obama's budget plan, the total U.S. debt would reach $16.2 trillion by the end of the year and soar to $25.4 trillion at the end of a decade's time.

"America can have robust economic growth ? and avert a catastrophic debt crisis ? but it requires a credible fiscal plan and the leadership necessary to achieve it," said top Senate Budget Committee Republican, Jeff Sessions of Alabama.

The government is likely to reach its borrowing cap ? the subject of a fierce fight last summer between Obama and Republicans ? late this year or early next year, which is going to require the next Congress and either Romney or Obama to act together to increase the borrowing cap. That is seen by many as an opportunity to force lawmakers to finally tackle the country's major budget problems.

Also looming is the so-called fiscal cliff, a combination of big tax increases and deep, automatic budget cuts slated to begin in January unless Obama and congressional Republicans step in to block them this fall or during a postelection session of Congress. Many economists say that if the government plunges over the fiscal cliff it could drive the economy back into recession.

Romney, for his part, offers relatively few specifics on the budget but promises to bring total government spending down to 20 percent of the U.S. economy by the end of a first term in 2016. That is roughly in line with where it was during Republican George W. Bush's presidency. Government spending now equals 24 percent of gross domestic product.

PESHAWAR?-?The administration has yet to take steps to seal offices of 27 private security agencies in accordance with directives of the Khyber Pakhtunkhwa Home and Tribal Affairs Department.

Through some highly-placed official sources, Pakistan Today came to know on Friday that a couple of days ago, the Home Department cancelled licences of 27 private security agencies.

In this respect, the Home Department directed the administration of various districts to sea offices of all the private agencies. However, the district administrations failed to implement the directives.

The sources said around 70 private security agencies were functioning in the province and 27 of them had failed in getting their licences renewed therefore on these grounds, the licences of these agencies were declared cancelled by the Home and Tribal Affairs Department.

The Khyber Pakhtunkhwa government has already initiated steps for streamlining affairs of private security agencies. It did not allow them to work with foreign directors. Similarly, the government has directed the Home and Tribal Affairs Department to compile data of all employees of the private agencies.
